Classify what the output is doing
A generated response may mix factual claims, calculations, quotations, interpretations, recommendations, and creative language. These require different checks. A fictional name in a story is not an error; an invented citation in a legal briefing is.
Underline the claims that carry the decision. Record dates, quantities, names, rules, sources, and assumptions. Ask what evidence would support each one and whether the claim can change over time, by jurisdiction, product version, or individual circumstance.

Match effort to consequence
A low-stakes brainstorm can tolerate imperfect suggestions that a person easily rejects. Health, legal, financial, employment, safety, security, public communication, or irreversible actions need authoritative current evidence and appropriately qualified human review.
Consider reversibility and reach as well as topic. A small internal draft may be corrected; an automatically sent message or published accusation can spread before review. When consequences rise, narrow automation, add approval, document sources, and make recovery possible.

Verify outside the answer
A second generated answer can reveal disagreement but is not independent confirmation. Follow citations to the actual source, check that it exists, read the relevant passage and scope, and compare primary or authoritative evidence with credible independent analysis where useful.
If evidence conflicts, is stale, or cannot be found, retain that uncertainty. Do not convert absence into confidence. Change the statement, seek an expert, obtain more evidence, reduce the decision, or stop the action.
